Job Summary
We are seeking a compassionate and dedicated Personal Support Worker (PSW) to provide exceptional care and assistance to individuals in various settings, including home, community, assisted living, and long-term care facilities. Duties

- Assist clients with Activities of Daily Living (ADLs) such as bathing, dressing, grooming, and mobility support using equipment like Hoyer lifts or transfer belts
- Monitor patient vital signs including blood pressure, pulse, temperature, and respiration; document findings accurately in compliance with HIPAA regulations
- Support medication administration following care plans and protocols to ensure safe and effective treatment
- Prepare nutritious meals tailored to individual dietary needs and assist with meal consumption when necessary
- Conduct patient observation to identify changes in condition or behavior, especially for those with dementia, Alzheimer's, autism, or developmental disabilities
- Maintain a clean and safe workplace by performing light housekeeping tasks such as cleaning patient areas and laundry




- Collaborate with healthcare professionals to develop and update personalized care plans that reflect each client’s unique needs

Skills

- Proven experience in home & community care, assisted living, nursing homes or long-term care environments
- Knowledge of patient care techniques including assisted mobility, medication management, and vital signs monitoring
- Familiarity with specialized care such as Alzheimer’s/dementia care, memory care, hospice support, and working with individuals with disabilities or developmental challenges
- Certifications in First Aid and CPR are required; additional certifications like HIPAA training are preferred
- Ability to perform heavy lifting safely using equipment like Hoyer lifts; excellent patient observation skills for early detection of health issues
- Strong communication skills to effectively interact with clients, families, and healthcare teams
- Demonstrated understanding of infection control practices and maintaining confidentiality in accordance with HIPAA regulations
