The Licensed Practical Nurse Sunnyhill in collaboration with a registered nurse (RN) provides care to patients with known health problems and predictable include assessing planning implementing and evaluating nursing care within a family centred interdisciplinary clinical framework

Learn more at and identifies patients and families actual potential strengths and limitations using physiological psychological social-cultural and spiritual needs by performing duties such as completing a nursing history and performing a head to toe assessment.

to the development and modification of individualized nursing care plans incollaboration with the RN and input from the family by performing duties such as formulating actual and potential patient problem lists identifying short and long term goals providing input to the RN regarding prioritizing care identifying nursing interventions and evaluating and monitoring patient and family responses to nursing interventions.

and collaborates in planning and coordinating the nursing care of patients by documenting and updating information about the patients condition reporting problems and/or potential problems to an RN and /or physician and changing the patients condition as required and completing admission teaching discharge and transfer sheets.





and interprets the established nursing plan of care provides nursing care and assists patients with hygiene grooming toileting needs ambulation and mobility and feeding and performs procedures such as dressing changes and medication administration to assigned patients according to established policies and procedures.

diagnostic treatment plans including collecting specimens and preparing and caring for patients undergoing diagnostic tests examinations or procedures.

in and supports patient and family teaching to patient care and discharge in the orientation of current LPN staff and LPN students and assists in the orientation of other members of the health care team.

Qualifications:

Education Training and Experience

Graduation from a recognized program for Practical Nurses or an equivalent combination of education training and experience. Current full practicing licensure with the British Columbia College of Nurses & Midwives (BCCNM).

Skills and Abilities

Ability to communicate effectively both verbally and in writing.
Ability to deal with others effectively.
Physical ability to carry out the duties of the position.
Ability to organize work.
Ability to operate related equipment.
