The role will include:

- Collaborating with the patient, family/caregiver, and interdisciplinary healthcare team to formulate an individualized plan of care based on thorough patient assessment
- Evaluating the effectiveness of treatment plans and adjusting care in consultation with the patient, family/caregiver, and healthcare team
- Acting as a strong patient advocate by protecting and promoting patient rights, dignity, and autonomy, especially in situations where the patient may be vulnerable or unable to advocate for themselves
- Applying critical thinking and clinical judgment to recognize and respond to changes in patient status, drawing on critical care experience where applicable
- Navigating complex family dynamics with professionalism and empathy, supporting families through difficult caregiving decisions while keeping the patient's best interests at the centre of care
- Managing responsive behaviours associated with dementia and cognitive decline, using de-escalation techniques to maintain a calm, safe environment for patients, families, and self
- Traveling within your region between patients’ homes
- Providing care across a variety of settings, which may include private homes, clinics, retirement homes, or schools
- Remaining comfortable and professional when entering homes that may be cluttered, under-resourced, or otherwise not in ideal condition, and adapting care delivery to the environment while maintaining safety and infection control standards
- Performing other clinical duties as required
- Delegating tasks as appropriate and within your scope of practice

Assets:

- Experience with wound care, IV medications and re-sites, catheter care, compression therapy/VAC, and ostomy care
- Critical care background and/or experience managing complex, high-acuity patients
