Reporting to the Director of Care or designate, the LPN leads an assigned community and coordinates care delivery. Duties include individualized care planning, wound treatment, prescribed medications, and resident monitoring. The position supports holistic wellness and residents’ rights.

Key Responsibilities
Assess residents and implement individualized resident-centered care plans Assess and treat wounds while supporting skin breakdown prevention Administer prescribed medications and treatments within BCCNM limitations Monitor, document,

Requirements:
