Help patients access addiction treatment at CATC through clinic administration, appointment coordination, and effective communication with healthcare professionals.

As a Float Patient Care Coordinator, you will provide general support within Canadian Addiction Treatment Centres’ outpatient clinics. The position requires understanding addiction and co-occurring disorders, maintaining confidentiality, and helping de-escalate or stabilize patients when needed. You will communicate with residents, families, referents, coworkers, physicians, counsellors, and pharmacy teams.

Key Responsibilities:

- Respond to patient and family inquiries across communication channels
- Manage scheduling, assessments, data, and documents in Accuro
- Follow up with patients about scheduled appointments




- Communicate patient needs and status changes to care teams
- Assist with office administration and supply ordering

Requirements:

- Customer service or vulnerable-population experience is an asset
- Experience billing through EMR is a definite asset
- Recent acceptable Criminal Reference Check
- Excellent interpersonal and communication skills
- Ability to learn quickly and solve problems effectively

Use sound judgment, confidentiality, and patient-focused communication while coordinating care across CATC’s multidisciplinary outpatient clinic team.
