Ross Memorial Hospital in Lindsay, Ontario, is seeking a Social Worker to join the Patient Flow team. The role supports protected, timely transitions from hospital to community, applying psychosocial assessment, discharge planning and advocacy within the Hospital to Home program.

The successful candidate will have a BSW, current OCWSSW registration, and hospital social work experience. MSW and OASW membership are preferred; must work Monday to Friday in a quick-paced health-care setting in Lindsay.
