Support housing stability for chronically homeless participants in Durham Region through landlord engagement and housing-based case management. This position includes telephone and home visits.

The Housing Specialist assists individuals and families with obtaining and maintaining appropriate accommodation. You will research housing options, facilitate assessments, coordinate referrals, and provide time-limited Critical Time Intervention case management. The work also includes advocacy, landlord mediation, crisis intervention, and rapid rehousing support.

Key Responsibilities:
Research housing options and maintain acceptance criteria
Liaise with landlords about vacancies and service agreements
Assist participants with applications, viewings, and transportation




Provide housing advocacy, referrals, and landlord mediation
Prepare case notes and statistical reports in HIFIS

Requirements:
College diploma in Social Sciences minimum
One to three years supporting chronic homelessness
Knowledge of mental health and addictions
Valid unrestricted Class G Driver's License
Current acceptable Vulnerable Sector Check and Driver's Abstract
