Working shifts scheduled by the Relief Coordinator, you will support pharmacy services throughout an assigned territory. Your work centers on disease state management, health risk assessments, medication adherence, prescription safety, and documentation. You will also provide professional customer service through Sobeys Inc.’s Canadian pharmacy banners, including Safeway, Thrifty Foods, Foodland, Lawtons Drugs, and FreshCo.

Key Responsibilities:

- Collect patient, drug, and disease information
- Develop care plans and document follow-up activities
- Monitor medication adherence and patient understanding




- Tailor counselling to literacy, hearing, privacy, and other concerns
- Coach pharmacy students and assistants when applicable

Requirements:

- Fluency in French and English
- Strong written and verbal communication abilities
- Organized, detail-focused approach to pharmacy services
- Self-direction while meeting strict deadlines independently
- Ability to provide skilled, patient-focused care
