Community nursing at ParaMed combines patient assessment and care planning with scheduled visits throughout County Area, Erie St. Clair District. Support patients across ages and care complexities in their homes.

As a Registered Nurse, you will provide care in various settings while helping patients remain comfortable at home. You will assess individualized needs, execute care plans, and monitor outcomes continuously. Building rapport and trust supports team-oriented patient engagement and therapeutic relationships.

Key Responsibilities:
Assess individualized patient needs using appropriate tools and techniques
Execute and continuously monitor individualized care plans
Build rapport and trust through cooperative patient engagement




Maintain and update qualified patient encounter records
Document plans related to patient care

Requirements:
Current CNO registration as a Registered Nurse in good standing
Valid driver’s license and access to a vehicle
Community care experience considered an asset
Reliability, flexibility, and willingness to travel
Required after-hours on-call coverage for patient safety
