St. Joseph’s Home Care seeks a Social Worker for its Integrated Comprehensive Care Program. This regular, part time position supports adults discharged home who need short-term clinical care. Apply counselling, discharge planning, service coordination, and advocacy to help clients and families navigate changing care needs.

Key Responsibilities:

- Conduct comprehensive psychosocial assessments from a systems perspective
- Provide counselling, crisis intervention, and illness adjustment support
- Coordinate complex discharges and develop protected discharge plans




- Connect clients and families with community services
- Document interventions and maintain client charts

Requirements:

- Bachelor’s Degree in Social Work from an accredited university
- Registration with Ontario College of Social Workers required
- Minimum one year recent related experience
- Home or community care experience preferred
- Willingness to travel throughout HNHB boundaries
