Oak Valley Health is seeking a Registered Practical Nurse (RPN) with a strong focus on stroke and rehabilitation care. You will perform comprehensive assessments, plan care, educate patients and coordinate discharge planning as part of an interprofessional team to ensure continuity of care during hospital stays.

Your clinical expertise will support development of patient-centered plans and collaborative programs, with emphasis on quality care and safety in a fast-paced workplace.
