Royal Free London NHS Foundation Trust seeks a Care Navigator for its Enfield Integrated Proactive Care Service. You will assess referrals, connect residents with health, social care, voluntary, and community services, and support person-centred care. The post contributes to integrated care planning and case management.

Key Responsibilities:

- Process referrals from GPs and multidisciplinary team members
- Assess needs against referral criteria and GP guidance
- Develop personalised support plans and share relevant information
- Coordinate referrals with healthcare, social care,



and voluntary professionals
- Identify urgent situations and alert appropriate professionals promptly

Requirements:

- Knowledge of local services and eligibility criteria
- Understanding of community, statutory, and voluntary support options
- Ability to identify unpaid carers and relevant support
