This experienced Registered Nurse supports clients through coordinated, quality home care. Reporting to the Care Manager, you will assess needs, develop care plans, and coordinate resources. The work includes collaboration with clients, families, and interdisciplinary teams across the continuum of care.
Key Responsibilities: Coordinate quality care for clients in the community
Identify client needs through solid nursing assessments
Facilitate internal and external resources with interdisciplinary teams
Coordinate and modify care plans as needed
Provide referrals or direct care when required
Requirements: Completion of an accredited nursing education program
Active CRNA registration and practice permit
Current Basic Cardiac Life Support Health Care Provider certification
Excellent communication and interpersonal skills
Flexibility for relief rotations, on call, and assignments
