This Registered Nurse position serves clients through evidence-based care tailored to their unique needs. You will assess clients, develop and evaluate care plans, and implement appropriate interventions. The role also includes coordinating transitions and connecting clients with local community support services. SE Health provides paid training, mentorship, and leadership support for practice to full scope. Current Ontario nursing registration and CPR certification are required.
Key Responsibilities: Complete comprehensive client assessments
Develop individualized evidence-based care plans




Implement and evaluate client care
Lead care for designated client caseloads
Connect clients with community support services
Requirements: Current Certificate of Competence with the College of Nurses of Ontario
Availability for days, evenings, nights, and alternating weekends
Updated immunization and required background check records
Current CPR certificate
Willingness to attain required SE Health certifications
Bring assessment, care planning, and collaboration skills to deliver client-focused nursing care with SE Health across Canada.
