As a Case Manager and Community Mental Health Clinician, you will support protected, timely, and effective discharge to the community. The position requires clinical experience in Mental Health and Co-Occurring Addictions, strong engagement skills, and family support expertise. Days and evenings are scheduled, with flexible hours including weekends available.
Key Responsibilities: Provide patient-centered care within Mental Health Ambulatory Services
Support safe, timely, effective discharge to the community
Apply therapeutic interventions with challenging family dynamics




Provide family support and education individually and in groups
Document care and maintain patient confidentiality
Requirements: Allied professional discipline, including MSW, OT, or Recreation Therapy
Membership in good standing with the appropriate regulatory body
Five to seven years recent related clinical experience required
Group experience and Intensive Case Management familiarity
Valid driver's license and access to a vehicle
Use mental health clinical expertise, therapeutic interventions, and community experience to support recovery-focused care at Joseph Brant Hospital.
