Provide skilled nursing care as a Registered Practical Nurse with AgeCare, supporting residents through individualized care planning and direct care.
Reporting to the Health and Wellness Manager, this regular part-time position supports residents in a retirement home setting. You will assess, plan, implement, and evaluate care while supervising health care aides. The role requires collaboration with multidisciplinary professionals and communication with relatives about resident care and condition.
Key Responsibilities: Assess individualized physical and emotional resident needs
Develop and maintain appropriate resident care plans
Supervise health care aides and provide direction
Communicate with relatives regarding resident care
Support discharge planning, transfers,



and resident teaching
Requirements: Current College of Nurses of Ontario competence certificate
Registered Practical Nurse program with medication administration training
Knowledge of geriatric, rehabilitation, or long-term nursing
Knowledge of applicable retirement home and health legislation
Ontario or evaluated equivalent medication administration certification
Bring resident care planning, nursing judgment, and collaborative communication to AgeCare’s Registered Practical Nurse team.
