About the Role
ComForCare Home Health Care - Brantford is looking for a compassionate and dedicated Registered Nurse to join our growing team in Brantford, ON. This is a rewarding prospect to make a meaningful difference in the lives of patients by delivering high-quality, personalized care right in the comfort of their own homes.

Responsibilities

Conduct comprehensive patient assessments and develop individualized care plans

Administer medications, treatments, and medical procedures in accordance with physician orders

Monitor and document patient health status, progress, and changes in condition

Coordinate care with physicians, families, and interdisciplinary health care teams

Provide patient and family education on health management, medications, and care techniques

Perform wound care, IV therapy, catheter care, and other skilled nursing interventions

Ensure compliance with all regulatory standards and home health care best practices

Requirements





Current Registered Nurse (RN) registration with the College of Nurses of Ontario (CNO)

Previous home health, community, or long-term care nursing experience preferred

Strong clinical assessment and critical thinking skills

Excellent communication and interpersonal skills for patient and family interactions

Ability to work independently and manage a caseload with minimal supervision

Valid driver's license and reliable vehicle for home visit travel

Current CPR and First Aid certification
