Community outreach and health education define this full time, in-person position with Quest Community Health Centre’s Urgent Service Access Team.
The Community Outreach Worker supports clients through social assistance programs and community services. Working within an interdisciplinary team, this position advances health promotion, disease prevention, community capacity building, and harm reduction. Quest CHC provides holistic primary health care throughout Niagara.
Key Responsibilities: Provide outreach to clients throughout Niagara communities
Deliver health education supporting informed client decisions
Administer social assistance programs and community services
Implement services addressing social determinants of health
Provide client follow-up within an interdisciplinary team
Requirements: Full-time availability for a 12-month contract
Ability to work 37.5 hours weekly
Ability to work in person
Commitment to harm reduction principles
Focus on client-centred, holistic community care
Support Quest CHC clients through outreach, health education, harm reduction, and community-based health services.
