St. Joseph’s Home Care seeks a Social Worker for its Integrated Comprehensive Care Program. This regular, part time position supports adults discharged home who need short-term clinical care. Apply counselling, discharge planning, service coordination, and advocacy to help clients and families navigate changing care needs.
Key Responsibilities: Conduct comprehensive psychosocial assessments from a systems perspective
Provide counselling, crisis intervention, and illness adjustment support
Coordinate complex discharges and develop secure discharge plans




Connect clients and families with community services
Document interventions and maintain client charts
Requirements: Bachelor’s Degree in Social Work from an accredited university
Registration with Ontario College of Social Workers required
Minimum one year recent related experience
Home or community care experience preferred
Willingness to travel throughout HNHB boundaries
