You will assess referrals, connect residents with health, social care, voluntary, and community services, and support person-centred care. The post contributes to integrated care planning and case management.
Key Responsibilities: Process referrals from GPs and multidisciplinary team members
Assess needs against referral criteria and GP guidance
Develop personalised support plans and share relevant information
Coordinate referrals with healthcare, social care, and voluntary professionals




Identify urgent situations and alert appropriate professionals promptly
Requirements: Knowledge of local services and eligibility criteria
Understanding of community, statutory, and voluntary support options
Ability to identify unpaid carers and relevant support
Ability to support care planning and case management
Ability to communicate across agencies and qualified groups
