Support daily living, patient safety, and wellbeing as a Health Care Aide with Consort Community Health Home Care in Consort. This casual relief role includes wide-ranging shifts.

As a Health Care Aide, you will support patients, clients, and residents requiring short term assistance or ongoing support. Working with nurses and multidisciplinary professionals, you will follow care plans and facility policies. Your work spans home, community, and health care facilities while engaging patients, families, and caregivers.

Key Responsibilities:

- Support activities of daily living according to care plans
- Assist nurses with hygiene, nutrition, mobility, and safety
- Support organized activities and recreation programs
- Observe and report patients' mood, health,



and wellbeing
- Maintain safe environments for patients, staff, and visitors

Requirements:

- Active or eligible Health Care Aide registration with CLHA
- Proof of education or Health Care Aide Certificate completion
- Complete certificate requirements within 18 months of hire
- Driver's license and vehicle required
- Satisfactory criminal record or Vulnerable Sector check required
