SE Health in Canada is hiring agile Physiotherapists to join the Acute Care Transitions Program, delivering client-centered home and community rehab. You will assess, plan and coordinate care with a diverse interdisciplinary team to support patients moving from hospital to home.

This hybrid role covers Richmond Hill and surrounding areas, with hours roughly 9:00am–9:00pm Mon-Fri and every other weekend. A valid PT license, ability to travel, and commitment to evidence-based practice are
