Key Responsibilities: Provide client-centered care in community settings
Support clients transitioning from hospital to home
Participate in joint care planning
Address physical, cognitive, social, emotional, and spiritual needs
Deliver accountable care and services to assigned clients
Requirements: Valid Personal Support Worker certificate, RN, or RPN
Valid driver’s license and vehicle for daily work
Current Standard CPR level C certificate
Current vulnerable sector check
Availability for days, evenings, and every other weekend
