Key Responsibilities: Provide nursing services to patients, families, and populations
Coordinate care in partnership with clients and families
Assess clients in residences or alternate care settings
Support patient safety, well-being, and health promotion
Provide client-centered care through nursing processes
Requirements: Accredited nursing education program completion
Active or eligible CRNA registration and practice permit
Current BCLS-HCP certification
Two or more years of Home & Community Care experience
Driver's license and required vehicle
