Key Responsibilities:

- Assess clients and determine service eligibility
- Develop and adapt individualized care plans
- Coordinate nursing and other client services
- Deliver direct care in community settings
- Support clients navigating available services

Requirements:

- Current Registered Nurse registration with BCCNM
- One year recent related clinical experience
- Experience supporting complex chronic health conditions
- Valid British Columbia Driver's License
- Knowledge of community health nursing practice
