Vaughan Community Health Centre seeks a Family Physician for permanent, full-time practice at 9401 Jane St. This clinician serves clients of all ages and families through health promotion, disease prevention, education, screening, referrals, and follow-up. Care is delivered in person and virtually for local and underserved communities.

Key Responsibilities
Provide primary care to individuals of all ages and families Perform histories, advanced assessments, diagnosis, and treatment planning Deliver clinic, community, home-visit, urgent,



and on-call services Coordinate hospital and community healthcare during client transitions Maintain records and support quality improvement strategies

Requirements:
Medical degree and recognized family medicine residency CCFP certification and Ontario registration in good standing Two to five years’ primary care experience, including residency Proficiency with EMR systems and Ontario healthcare technology Experience providing culturally and linguistically responsive care Bring family medicine expertise, interdisciplinary collaboration, and culturally secure primary care to Vaughan Community Health Centre clients. #J-18808-Ljbffr
