**NURSE PRACTITIONER — Integrated Health | Keswick, Ontario**

Something amazing is coming to Leswixk An integrated health faciltiy in Keswick and we are looking for a Lead Nurse Practitioner who wants to practise at the top of their scope — and help shape what whole-person care looks like in this community.

Here, patients don't bounce between disconnected providers. They get a team that actually talks to each other and plans a treatment protocol to add to their current management team. As our Nurse Practitioner, you're the medical anchor of that team — the clinician who connects the dots.

**What you'll do**

- Consuct assessment and have in-depth discussions with patients about their health journey and where they would like to see it go

send for medical testing as required especially blood work prescribe ecg and order and interpret diagnostics, within the full RN(EC) scope

- Build longitudinal relationships with patients, not 10-minute transactions

- Collaborate daily with allied health practitioners on shared care plans

- Lead on chronic disease management, prevention, and healthy aging — including a large and growing midlife women's health population





- Help design new programs and services integrating other practioners in the clinic and be familiar with allied Health interventions such as iv treatments

**What you bring**

- Registered Nurse in the Extended Class NP in good standing with the College of Nurses of Ontario

- A genuine team mindset — you like working alongside other disciplines, not in a silo

- Strong clinical judgment and the confidence to practise autonomously

- An interest in integrative, root-cause care (not just symptom management)
