Our approach combines medical assessment, individualized treatment plans, preventative health strategies, and ongoing patient monitoring to support sustainable, long-term health outcomes.

We are seeking a qualified Nurse Practitioner to join our team and contribute to the development of a patient-centred practice focused on personalized healthcare and human optimization.

Key Responsibilities

- Conduct comprehensive patient assessments, medical histories, and clinical evaluations.
- Order and interpret laboratory investigations within the NP’s authorized scope of practice.
- Develop individualized treatment plans based on clinical findings, patient needs, and current evidence.
- Assess and manage appropriate patients seeking hormone-related healthcare and metabolic health support.
- Provide medical weight management services, including assessment for appropriate pharmacological treatments.
- Prescribe medications when clinically indicated and authorized.
- Monitor patient progress, treatment effectiveness, laboratory markers, and potential adverse effects.
- Educate patients on nutrition, exercise, lifestyle interventions, and preventative health.
- Review body composition assessments and relevant health data to support clinical decision-making.




- Maintain accurate patient records and ensure compliance with applicable privacy legislation and professional standards.
- Coordinate referrals and collaborate with other healthcare professionals when appropriate.
- Contribute to the development of clinical protocols, treatment pathways, and patient care processes.
- Provide in-person and, where appropriate, telehealth consultations.

Qualifications

- Current registration in good standing with the College of Nurses of Ontario as a Nurse Practitioner.
- Appropriate authorization to practise and prescribe in Ontario.
- Robust clinical assessment, diagnostic reasoning, and patient communication skills.
- Experience in primary care, family practice, preventative health, or a related clinical setting.
- Commitment to evidence-based medicine, patient safety, and ethical clinical practice.
- Ability to work independently while recognizing when consultation or referral is necessary.

Experience in hormone-related healthcare, medical weight management, obesity medicine, or metabolic health is considered an asset.
