Nurse Practitioner Opportunity - West Mountain Family Physician Office in Hamilton

Job Summary

We are seeking a motivated and patient-focused Nurse Practitioner (NP) to join our established Family Medicine Office on Hamilton's West Mountain.

Our office consists of three physicians who are committed to providing comprehensive, high-quality, patient-centred care to our community.

Position Details

Location : West Mountain, Hamilton, Ontario

Schedule: 3 days per week from 9:00 am to 5:30 pm

Practice: Established Family Medicine Office with three physicians

Duties

- Provide comprehensive Primary Care assessments and follow-up.
- Assess, diagnose and manage acute and chronic medical conditions within scope of practice.
- Provide preventative care, health promotion and patient education.
- Order and interpret appropriate investigations and prescribe medications within scope.




- Collaborate with Family Physicians and other Healthcare Professionals to support continuity of care.

Qualifications
Current registration in good standing with the College of Nurses of Ontario as a Nurse Practitioner. Primary Care or Family Practice experience preferred. Strong clinical assessment, communication and organizational skills. A team-oriented, patient-centred approach to care.
