Job Summary
We are seeking a dedicated and organized Resident Care Coordinator to join our team.

Responsibilities

- Monitoring Residents Health/Well-being
- Responsible for overseeing and managing care provided to residents, ensuring resident documentation is appropriate and care plans are up to date
- Supervise and train new hires, along with ensuring new hire documentation is received
- All Floor Staff (Personal Care Workers) report directly to Resident Care Co-Ordinator On-Call Staffing Support; receiving and covering sick calls/vacation, etc.
- Ensure floor staff are adhering to NL Health Services guidelines/protocols- Including reporting to management if Delegation of Function is required
- Provide ongoing support and guidance to staff
- Coordinate with Health Care professionals re: care needs, wound care, etc.
- Report to manager any information requiring escalation
- Act as a primary point of contact for residents and families
- May assist in payroll when manager is absent
- Coordinates Doctor's Appointments for residents who see PCH assigned physician




- Coordinate day-to-day medical appointments/travel requirements
- Provide Manager with required information to complete month end reports
- Ensure Appropriate Documentation of Medication Administration Record (MAR)

Experience

- Previous experience is required.
- Familiarity with Personal Care Home or medical environment would be considered an asset.
- Strong organizational skills and attention to detail are necessary for maintaining accurate resident information.
- Excellent interpersonal skills to foster positive relationships with residents and staff alike.
