UHN seeks a Personal Support Worker for 8B GIM, working within an inter-professional care team. The role supports feeding, transfers, bathing, documentation, and communication. Assigned tasks follow RN or RPN delegation, completed education, training, and RHPA requirements.

Key Responsibilities:

- Assist patients with feeding, lifts, transfers, bathing, and toileting
- Perform delegated tasks under RN or RPN oversight
- Communicate information according to established patient care plans
- Report unsafe conditions and patient changes to regulated providers
- Complete patient records and progress notes according to policy

Requirements:

- Current registration with the HSCPOA
- Current Basic Cardiac Life Support certificate
- Ability to manage daily mild to moderate physical exertion
- Acute care or previous rehabilitation experience considered an asset
- Good attendance record is required
