Duties: Performs professional community health nursing work across the life span by carrying out community health programs and services in homes, schools, clinics, and other community settings.



Assists and/or conducts assessment of community health needs and assets; provides nursing care and health promotion activities; and liaises with other community and professional personnel. Provides programs and services from preconception to school entry and conducts school health programs including a school health assessment, a provincial immunization program, and communicable disease programs. Provides acute, chronic and palliative nursing care services; monitors personal care homes; conducts provincial home support program and special assistance programs; and utilizes the assessment and placement single-entry process.
