Job : The Case Manager provides social case management support as a primary care provider to the client population. The population targeted for this project are those who have difficulty engaging or have disengaged from care, involved in substance abuse and who are impacted by substance use disorder and Mental Health issues. "1 year experience working with marginalized population in a health-related field

Bachelor of Social Work (BSW)

Basic - Variety of computer programs

- Knowledge in developing policies, procedures and standards
- Ability to be flexible and willing to change
- Knowledge in the SHA, community resources and applicable programming
