The role supports pregnant individuals, infants, children, and families facing varied health and social needs. You will deliver programs, workshops, and group sessions while building trusting relationships with families and community partners. Trauma-informed practice, traditional knowledge, and community values inform service delivery where appropriate.

Key Responsibilities:

- Deliver maternal and child health programs and workshops
- Build relationships with families and community partners
- Support families experiencing high-risk situations
- Assist with case planning and team meetings




- Maintain confidential records, statistics, and reports

Requirements:

- Related diploma or certificate in health or social services
- Knowledge of family systems and community referral pathways
- Training in trauma-informed care or prenatal support
- Ability to work independently and within multidisciplinary teams
- Valid driver's license for home visit travel, if applicable
