This full time Registered Nurse role supports women through menopause and related health concerns. You will provide virtual nursing, care coordination, health coaching, and evidence-based education. The work focuses on individualized care plans and sustainable health outcomes.

Key Responsibilities:
Serve as patients’ primary contact throughout their care journeys
Provide virtual case management through phone and digital platforms
Conduct nursing assessments and identify patient needs and barriers
Develop individualized care plans with healthcare providers




Coordinate referrals, follow-ups, and interdisciplinary team communication

Requirements:
Current Canadian Registered Nurse registration
Active licensure in at least two listed provinces
Minimum three years of recent nursing experience
Robust assessment, communication, and patient education skills
Experience using EMRs; EPIC strongly preferred
