Oak Valley Health seeks a Nurse Practitioner to promote health, prevent illness, and manage care for older adults. The NP collaborates with Markham Stouffville Hospital teams to support reactivation for Alternate Level of Care patients. Advanced knowledge of frailty, diagnosis, therapeutic intervention, and geriatric nursing is central to this work.

Key Responsibilities:

- Promote health and prevent illness, injury, and complications
- Lead care management for complex frail older adults
- Provide direct patient care and qualified consultation
- Promote excellence in geriatric nursing practice




- Support patient transfers and occasional MSH travel

Requirements:

- Master’s degree with Ontario College of Nurses registration
- RN(EC) Primary Health Care or Adult Nurse Practitioner registration
- Minimum five years’ current geriatric care expertise
- Competence in assessment, diagnosis, management, and intervention
- Proficiency with Word, PowerPoint, and Outlook
