Grand River Community Health Centre seeks an RPN for its Inter-qualified Primary Care Team. The position includes assessments, physical examinations, health screening, immunizations, injections, venipuncture, wound care, and chronic disease education. Care is documented in an EMR according to college requirements.
Key Responsibilities:
Conduct assessments, examinations, and health screening
Administer immunizations, injections, and provide wound care
Perform venipuncture and other in-scope nursing duties
Support chronic disease education for community clients
Coordinate care plans with teams and system partners
Requirements:
Current Ontario Registered Practical Nurse certification
Current CPR and First Aid certification
Preferred three to five years primary care experience
Robust communication, organization, and time management skills
Access to a car and required auto insurance
