As a Social Worker II, you will provide social work services within the Extended/Transitions program. You will support patients and families through psychosocial assessment, counselling, case management, and coordinated transitions. The position also includes group and family treatment, risk screening, resource-based interventions, and prevention activities. You will collaborate with an interdisciplinary team while reporting to the Clinical Operations Manager.

Key Responsibilities:

- Facilitate and coordinate client discharge plans
- Deliver group work services with the program team
- Provide psychosocial assessment and resource-based interventions




- Coordinate case management and patient transitions
- Support counselling, treatment, and public awareness activities

Requirements:

- Bachelor's degree in Social Work
- Active or eligible Alberta College of Social Workers registration
- Minimum one year mental health facility experience within three years
- Communication, organization, and group therapy facilitation skills
- Microsoft Office, internet, email, and case management experience
