At the Claresholm Centre for Mental Health and Addictions, you will provide social work services within an interdisciplinary team. The position serves patients and families through psychosocial assessment, counselling, case management, and coordinated transitions. Your work supports extended and transitions programming across the Claresholm Care Facility.

Key Responsibilities:

- Facilitate and coordinate client discharge plans
- Deliver group work services with the program team
- Provide psychosocial assessments and resource-based interventions
- Coordinate case management and transition planning
- Support individual, group, and family treatment

Requirements:

- Bachelor's degree in Social Work
- Active or eligible Alberta College of Social Workers registration
- Minimum one year mental health facility experience within three years
- Working knowledge of mental health disorders
- Communication, organization, and group therapy facilitation skills
