Fraser Health seeks a Licensed Practical Nurse, Full Scope, for Home Health in Port Moody, B.C. This community-based position manages assigned caseloads and supports clients with complex chronic conditions. The work includes direct care, health counseling, transitions, and collaboration with interprofessional providers.

Key Responsibilities:
Establish therapeutic relationships respecting client autonomy and privacy
Assess clients and develop individualized interprofessional care plans
Provide direct care, counseling, education, and self-management support
Coordinate transitions across primary, community,



and acute care
Maintain clinical records and participate in quality improvement

Requirements:
Current practicing BCCNM registration as a Licensed Practical Nurse
One year recent related clinical experience
Experience with complex geriatric or adult chronic conditions
Valid British Columbia driver's license and personal vehicle access
Ability to communicate effectively verbally and in writing
