Jean Tweed Centre in Milton, Ontario seeks a qualified Case Manager to support women facing mental health, substance use and criminal-justice involvement. You will develop holistic, client-centred service plans, coordinate wrap-around supports, provide counselling and outreach, and navigate referrals to health and community resources.
The role is hybrid, 37.5 hours/week, with in office work at Milton locations and one remote day.
