Deliver organized community home care as an RPN/LPN, using assessment, care planning, and nursing standards to support patients and residents.
Extendicare seeks a part-time Registered Practical Nurse or Licensed Practical Nurse to strengthen daily nursing practice. The work includes observing conditions, documenting findings, reviewing care plans, and responding to changes requiring medical referral. A reliable vehicle is required for community home care assignments.
Key Responsibilities: Observe patient conditions and document vital signs and symptoms
Assess patients or residents with nursing support
Develop, implement, review, and evaluate care plans
Recognize changes and assess requirements for doctor referrals
Follow established techniques, policies,



and physicians’ orders
Requirements: Provincially approved current certificate of competence
Current registration or eligibility with the approved institution
Previous long-term care or community homecare management experience preferred
Comprehensive nursing and healthcare practice knowledge
Solid organization, planning, time management, and multitasking skills
Use reliable clinical documentation, care coordination, and nursing judgment while supporting Extendicare’s community home care services.
