Build therapeutic relationships while preserving patient dignity and privacy. Develop care plans, provide nursing services, and communicate with interdisciplinary care teams.

Key Responsibilities:

- Develop effective care plans with Ontario Health atHome
- Provide nursing services in person and virtually
- Serve patients in homes, clinics, schools, or retirement homes
- Communicate with patients, families, and interdisciplinary teams
- Perform other clinical duties as required

Requirements:

- Registered Nurse or Registered Practical Nurse degree or diploma
- Current College of Nurses of Ontario registration
- Valid driver’s license and vehicle for regular travel
- Excellent English verbal and written communication skills
- CPR certification, immunization record, and required checks
