UHN seeks a Personal Support Worker for 8B GIM, working within an inter-qualified care team. The role supports feeding, transfers, bathing, documentation, and communication. Assigned tasks follow RN or RPN delegation, completed education, training, and RHPA requirements.
Key Responsibilities: Assist patients with feeding, lifts, transfers, bathing, and toileting
Perform delegated tasks under RN or RPN oversight
Communicate information according to established patient care plans
Report unsafe conditions and patient changes to regulated providers
Complete patient records and progress notes according to policy
Requirements: Current registration with the HSCPOA
Current Basic Cardiac Life Support certificate
Ability to manage daily mild to moderate physical exertion
Acute care or previous rehabilitation experience considered an asset
Good attendance record is required
