As a Social Worker II, you will provide social work services within the Extended/Transitions program. You will support patients and families through psychosocial assessment, counselling, case management, and coordinated transitions. The position also includes group and family treatment, risk screening, resource-based interventions, and prevention activities. You will collaborate with an interdisciplinary team while reporting to the Clinical Operations Manager.
Key Responsibilities: Facilitate and coordinate client discharge plans
Deliver group work services with the program team
Provide psychosocial assessment and resource-based interventions




Coordinate case management and patient transitions
Support counselling, treatment, and public awareness activities
Requirements: Bachelor's degree in Social Work
Active or eligible Alberta College of Social Workers registration
Minimum one year mental health facility experience within three years
Communication, organization, and group therapy facilitation skills
Microsoft Office, internet, email, and case management experience
Use mental health knowledge, clinical coordination, and collaborative communication to support patients and families at Claresholm Centre for Mental Health and Addictions.
