Support elderly clients through home-based case management, service planning, and community social services coordination. This full-time position requires 35 hours weekly and is not remote.

The Case Manager helps elders develop and maintain maximum independent living. Responsibilities include assessing needs, developing service plans and goals, conducting home visits, and maintaining exact records. The position also requires extensive contact with community social services agencies, strong writing skills, and sensitivity to elders’ needs.

Key Responsibilities:

Requirements:

- Full-time availability for 35 hours weekly
- Social work experience and social services knowledge preferred
- BA or BS in Human Services, Social Services, or related field preferred
- MA Social Work license desirable
- Valid driver’s license or reliable transportation and good driving history
