As a Social Worker II, you will provide integrated services within a multidisciplinary team at Sheldon M Chumir Health Centre. Your practice includes psychosocial assessment, treatment planning, crisis intervention, and transition support. You will work with clients experiencing severe and persistent mental illness, using recovery, harm reduction, trauma informed care, and client centered approaches.
Key Responsibilities: Complete intake assessments for Mental Health Housing programs
Provide psychosocial assessments and resource based interventions
Coordinate case management, treatment, and transition planning




Facilitate individual, group, and family treatment
Visit clients and transport them using a personal vehicle
Requirements: Bachelor's degree in Social Work
Active or eligible Alberta College of Social Workers registration
Three to five years adult mental health experience
Experience with severe and persistent mental illness
Valid driver's license and vehicle required for work travel
Support AMH Supported Living clients through mental health outreach, housing consultation, assessment, and coordinated social work services.
