Social Worker (Youth)

Job description
The WestView Primary Care Network is looking to hire a Youth Social Worker to cover an 18-month maternity leave. The maternity coverage is November 30, 2026, to May 31, 2028. The Youth Social Worker is a key role in running the Youth Mental Health Clinic and supporting youth in Stony Plain, Spruce Grove and Parkland County. The Youth Social Worker works closely with the Youth Mental Health Clinic physicians, the Westview PCN Adult Mental Health team, Kickstand, Stony Plain Youth Centre, Evergreen School Division and Parkland School Division.

Key Responsibilities
The Youth Social Worker role is expected to be flexible and accommodating to the needs of youth ages 11-24 and their families.
- To work collaboratively in a youth centered, integrated team based care delivery system
- To participate in the care of individual patients
- Read and assess referrals for approval or to escalate them to a higher level of care
- Complete intake assessments with youth who present to the clinic
- Complete referrals to outside sources for services
- Provide social services and connections for youth
- Complete letters for court and/or income support
- Organize and run team conferences after each clinic
- Create risk assessments and safety plans for those youth who present in crisis
- Develop and communicate brief intervention strategies with referral sources
- To act as a resource and liaison to family physicians and other multidisciplinary team members,



healthcare institutions and community agencies or programs; contributing to service delivery planning including clinical practice and program development, evaluation, and stakeholder consultation
- Maintains excellent working relationships with other local mental health care providers to ensure appropriate referrals
- To ensure accurate, objective and timely clinical documentation in the appropriate medical record for both clinical and evaluation purposes
- Other duties and responsibilities as they emerge; versatile work schedule to meet urgent business needs and/or service requirements
- Willingness to support the vision and mission of the Parkland School District and the WestView PCN Position Requirements
- ACSW registration
- First Aid
- Training in Suicide Prevention and/or Crisis Intervention
- Experience working with youth in a professional manner
- Knowledge of Province of Alberta privacy laws
- Professional, accountable, excellent written and verbal skills.

Salary:
$28.00+ per/hr
