Provide individualized hospice care as a Registered Nurse with Brighton/Suncrest Hospice across patients’ homes and care communities. Use communication and collaboration daily.
This Registered Nurse role serves patients in private residences, skilled nursing facilities, and assisted living communities. You will develop care plans with nurses, Social Workers, Chaplains, CNAs, and volunteers. The work supports physical, emotional, and spiritual needs while requiring independent scheduling, documentation, and urgent-needs management.
Key Responsibilities: Provide compassionate hospice care wherever patients call home
Develop interdisciplinary care plans for patients and families
Support physical, emotional, and spiritual patient needs
Manage daily schedules, visits, documentation,



and urgent needs
Utilize electronic medical record systems
Requirements: Current Registered Nurse license in good standing
At least one year of hospice experience
Solid interdisciplinary collaboration and communication skills
Reliable transportation, driver’s license, and current auto insurance
Ability to work independently and organize daily responsibilities
Bring hospice nursing, care planning, and communication skills to Brighton/Suncrest Hospice patients and families throughout the service area.
