Provide expert geriatric care as a Nurse Practitioner at SHN’s GAIN Clinic, supporting frail seniors through comprehensive assessment and intervention. This permanent full time position serves adults and seniors across Scarborough.

Working within an inter-professional team, the Nurse Practitioner manages acute and chronic illness, wellness promotion, and complex geriatric needs. The role includes outpatient clinic care, outreach consultations, and consultation liaison services for inpatient and emergency department populations. Flexible hours include evenings and weekends, with possible home visits.

Key Responsibilities:

- Lead comprehensive geriatric services through the GAIN Clinic
- Assess frail seniors and support health promotion, safety, and function
- Provide outreach consultations in homes and community settings




- Support care plans in long-term and retirement homes
- Advise inpatient and emergency department teams on complex geriatrics

Requirements:

- Master’s degree in Nursing
- Current Extended Class registration with the College of Nurses of Ontario
- Three years’ recent Nurse Practitioner experience with frail older adults
- Strong communication, interpersonal, organizational, and problem-solving skills
- Ability to exercise independent judgment and maintain confidentiality
